I'd say your latter scenario (leaky injector hose connections) is the likely culprit.  If the little hoses on the injectors have been replaced, the new connection may well be leaking.  Causes for a leak here include: 1) when the crimped ferrule was cut off, someone wasn't careful and cut into the injector ;2) the replacement hose is old and/or split or of the wrong size; and 3) the hose clamp is on either too loose or too tight.  

Use the 5/16" hose (it's just a smidge bigger than the original 7mm hose and is less prone to splitting when pushed over the fittings) with small worm-gear type hose clamps.  There might be better clamp alternatives out there, but worm gear clamps are easy to find.  Whatever kind of clamp you use, don't overtighten it or the connection will leak.  Go over the entire fuel rail to make sure you don't have any old, cracking, age-hardened rubber hose left in there.  

I guess you could also check the integrity of the injectors themselves by pulling the injector pairs and doing a visual check of the spray patterns per the instructions in the Bentley manual.
