As far as I can remember, yes, they did. This rubbery backing is to 
prevent squeeks.

I doubt it. I can not remember what kind of caliper the Volvo has, but I 
can easily find out tomorrow.

The anti rattle spring is there to keep the pads firmly located, I 
actually dont know about the piston locating plate.
