While this may have just been a random failure, I believe that the major cause of pulley failure is a bad voltage regulator. Old regulators tend to stick on and this puts a LOT of torque on the pulley, in sudden hard thumps. This pulls the belt down into the V-groove, wedging the pulley halves apart and breaking them. This sudden and abrupt changing in torque on the pulley might also be perceived as a rough idle. You could check this by removing the DF (NOT the D+) wire from the generator and see if this makes your idle settle down.
