Yes, I think these are still available. If you can't find them, I can rebuild 
it for you. The tandem MC is a worthwhile upgrade.

I have several good used ones.

It is just tight and corroded. Once the cable is out you can tap them loose 
with a hammer. Save any washers that are on there; you may need them.

4-bolt lug bolts are M14 x 1.5. I don't have any 5-bolt parts here.
