It's clearly an arbitrary choice which gets fixed by tradition. I'm actually surprised that you think there are 40. I'd be interested in seeing the list. Of all the countries that drive on the left, aren't all of them former parts of the British empire except for Japan? This makes it pretty clear where this got inherited. I don't know about Indonesia; I didn't know that they drove on the left, too.
