The key on your pulley -- the little knob that fits into the hole to
index it -- can easily go out of round, which puts your timing marks off
and allows the sort of vibration that can back the bolt out again, and
again, until the pin eventually shears off completely and you need a new
pulley. Meanwhile the car is running bad because your timing mark is no
longer accurate. And since the pulley is what holds the fan in place,
the fan can move around, too, causing *it* to loosen up its key, etc.
I'd plan on replacing the pulley at the first opportunity, and take a
good hard look at what's been going on with the fan.
