I have a approximately a 2" diameter  hole that both tubes come out
of, so, imagining this washer trick it seems like I would have to push
the accelerator tube to the side, and then the washer would only get
purchase on part of the opening. I suppose we could drill a side hole
in the washer for the accelerator tube.

This sounds totally wrong. Both tubes should exit out of separate, snug fitting
holes in that bulkhead, where they should have been individually tack welded
where they exited. THIS is the place that takes the lion's share of the force
when you push on the pedal. If this has been cut away (maybe someone converted
this from AT to MT) then it's no surprise that the other secondary welds broke,
as they were never meant to take all that stress.

How can I best identify if this was originally an automatic?

I'll try and get some photo's of the situation.
