I've heard of people who don't have reverse gear who passed the test but needed to be pushed back out of the testing centre - I think they've closed that loophole now. They can fail a headlamp with bad chrome on the reflector - the outer wouldn't be a problem unless a danger to other road users or pedestrians due to sharp edges etc. I don't think a non-functioning front foglight would fail in the UK, though they were making noises about aim of auxiliary lights, but you have to have a spec and measuring equipment for that.
