It sounds like you were pretty thorough, but SOMETHING was clearly wrong here. 
I don't have any clear cut answers, but here are some off-hand ideas for you to 
consider.

Did you use your timing light to verify that the mech advance was working?

Have you compared the Bentley manual with the cooling tin you have to make sure 
that it is ALL there? Spark plug air seals in place?

Did you look at the injector spray patterns to see if they were okay? [This is 
a long shot, but it seems like you were running lean for some reason.]

Were you using good quality exhaust valves (very important) with chrome plated 
exhaust valve stems, & bronze valve guides?

I almost never replace the oil coolers, but I DO clean out the air passages, 
just to make sure that the air can get thru there and do its job. The exception 
would be if the engine had destroyed internal parts that could have led to bits 
and pieces lodged in the cooler. In that case I would be tempted to replace it.

This seems awfully quick. There must have been something dramatically wrong.

"Plastic locknut insert"? Melted? Something was wrong here. Was the thermostat 
opening the flaps? Were the flaps frozen in place, or the thermostat not 
adjusted right? Thermostat dead?
