You are right; if you want to use one of these type 4 fans you have to suck air from inside the car and blow it through the heat exchangers (instead of the engine fan) for it to be advantageous. I had a very leaky bug that I drove back and forth from the Pittsburgh area to PSU back in the '80's and this set-up worked well. My Fasty's heat works well with the stock set but it is in a lot better shape than my bug was. Although, it is very nice to have that electric fan to help keep the windshield defrosted when you are stuck in traffic with four people in the car.
