Happens when the spring on the points is weak and or the cam in the distributer is turning so fast (high rpm) that the follower on the points bounces from lobe to lobe instead of smoothly opening and closing. Or look at a VW distributer and turn it by hand while watching the points open and close. Now picture the distributer turning so fast that the points don't have time to close... On a stock or mildly modified VW this is something you would probably never see unless the spring on the points was really weak...
