I'm not sure, but I think that once the rings start to gall, then metal can start to transfer back and forth between the ring and the cylinder, and this can lead to enough buildup that it starts to scratch the piston. A scratch in the side of the piston is really not much of a problem, unless the scratch involves enough metal next to the ring groove that it seizes up the ring motion. Then it is a problem.
