It is obviously some kind of clutch problem, so you will probably 
have to pull the engine to find/repair.  I would suggest that you 
start by watching the motion of the clutch arm where it goes into the 
bell housing just to make sure that is working okay.

Things to look for: throwout bearing cross shaft arms, cross shaft 
bearings, throwout bearing spring clips, pressure plate.
