Thanks for all the suggestions. I'll keep my fingers crossed that the lock
plate is worn out.

The locking tab may be too worn. Drill a small hole through the bolt head
and thread a wire through it. Wrap the wire around the torsion housing.
This will keep the bolt from loosening.

It was let go too long and the hole is rounded out. You need a new
stabilzer and arm to correct it

There is a set screw on the opposite (left) side. You may have to drill a
new dimple in the bar if it is stripped.

You MUST tighten the pinch bolt as well as the bolt that goes in the end
of the torsion bar. If the bar has been operated loose for a long time,
the hole in the upper arm will be augered out and no amount of tightening
will make it stay tight.
