The most common cause of this is exhaust leaks where the lower heat exchangers 
connect to the muffler. Leaks there will spray exhaust forwards, where it can 
squirt inside the heat exchanger and join with the heated air.

This can be difficult to fix, but you have at least 2 options: Seal the HE to 
muffler connection, or seal the sheet metal to exhaust pipe joint at the rear 
of the HE. Or you can try both.
