It's good sense to replace the fibre washer on the main jet access screw - it's in the kit, but there's nothing special about it. We tend to just re-use these things, and then wonder why they don't do the job they were designed for! I've not heard of it being a frequent problem - John's episode in Wyoming is the only one I know of. Fuel feed spigot falling out of the carb is much more common, but I've not had that happen either.
