My first thought on hearing this is that either the gearbox is moving on the 
beam, or the gearbox cover is moving on the gearbox. I would want to look 
carefully at the margins of the gearbox mounting for signs of motion. This 
could be because the gearbox was not tight enough, or it was tight, but there 
was dirt under it so that it did not actually fit securely.

I would also check to make sure that the Pittman arm is tight on the gearbox 
shaft and that the input flange is also tight. Those parts are all on splines, 
however, so not much motion would be possible.

I think the bolts thru the flex coupling pass thru 4 steel bushings that leave 
no room for play. You should check to make sure that none of these bushings 
were lost, and that all 4 bolts were tight.

You should also check the 4 locks on the tie rod ends.
