I have never dealt with these, but I always just assumed that the center spacer 
just floated there, and didn't need to be held in place. Now that you mention 
it I can see that this would leave one rocker with a double load on one side.

I think that even with a dimple in the shaft, it would be hard to keep this 
spacer in place and tight. It would probably need to be a very close fit on the 
shaft, or it would work its way loose eventually.
