It is a sign of wear on the shift fork, shift slider, and 4th gear, or which ever gear is involved. From what I have been able to figure out, a lot of time it is caused by the driver going down the road with his hand on the shift lever, applying pressure to the linkage. You might be able to help it with a shifter adjustment.
