Boy! I don't know how you could do this if the plugs are the right 
ones. You clearly know better than to put 3/4" thread plugs in your 
1/2" thread heads.

Is it possible that they get bent in just in the process of installing 
them? Try taking them out right away to see if you were able to get 
them in without bashing them on the side of the plug hole.

The only 2 ways I can think of would be to either be hit by the 
piston, or hit in the process of installing them. I don't think the 
former is possible even with a rod bearing gone as long as you 
have the right plug, and I don't think the latter is something that 
YOU would do.
