I understand that the air is recirculated when the temperature setting is on high. The temperature is regulated by adjusting the mix of recirculated air and cold outside air. CO is really not a problem since the system is independent (with the exception of the hoses for coolant coming through the firewall) of the engine compartment and even the best cars are not sealed that well and have built in vents. Plus one would seldom leave the temperature setting on high for very long (try it some time, it get really hot) and would therefore always be introducing some fresh air. The air conditioning works similarily. When on MAX A/C it is recirculating the air inside the passenger cabin rather than trying to cool the hot outside air.
