You need to file the following forms:

	Certificate of Non-Operation - Telling them where vehicle was stored.
Creates a vehicle that your trying to register, a first step. A California
vehicle that has not been registered for 4 yrs and the first step towards
abandonment to create new title. 

	Application for Original Registration - [If vehical hasn't been registered
for the past 4 years then you can create a new title for it under an
Abandonment clause.]

	Statement of Facts -  ie; History of vehicle.

All get submitted together at DMV.  Vehicle will have to be inspected and
title might not be received until it's California street legal.
