This sounds like there's a fuel delivery problem. My guess is a clogged filter, but don't guess, you can test for this. Check the fuel pressure while driving, using a long hose to the gauge, placed where you can see it. If the filter (or the filter screen) is clogged, then the pump will deliver enough gas to idle, but won't be able to deliver enough to supply the engine under load. I don't know how this correlates with your previous description of running fine TO work but failing halfway home in the evening, but I think your next step should be to do the "dynamic" fuel pressure test.
