Congratulations, this is not a rebuild problem. Now the question becomes: what's wrong with the fan? Off balance, installed wrong, or hub failure. And the smoke, of course, which has to be a separate problem.

Just to clarify: to "throw a rod" means that the rod actually disconnects from the crankshaft. This is indeed a catastrophic failure and can put a big hole in the casem but it doesn't happen often. The usual failure is that a rod journal will loosen or the bearing will wear such that a bearing half dislocates in the journal (to 'spin a rod' or 'spin a bearing'). This will usually sieze the motor; it stops. If it's really going fast when it seizes, you can break something, like the rod.

A rod will knock because one of its bearings is loose, usually at the crank, and when the cylinder fires the rod actually impacts the journal. This hammering can make things worse fast, but it comes before spinning, seizing or throwing anything.
