I've had a retainer come loose inside one and make a lot of noise until it got completely ground up. Sometimes it can just be a bit of loose rust. Do the brakes still work okay? If they seem spongy, look for a cracked drum, but I think this is rare; I've never seen one. Could it be a CV joint? Grab each axle and move it axially 1/2" each way a few times; this will redistribute the grease. See if that changes the noise while you drive.
