Bob Hoover rants about the crud that can build up in the corners of the oil 
passage drillings. He's one of several people who talk about the importance of 
cleaning out the passages whenever you rebuild. 

RIMCO will drill and tap all your oil galley plugs so that you can clean and 
verify them before reassembly. I've only ever done this to one engine which I 
rebuilt, but I wash out all my cases really, really well, flushing all the 
passages in both directions with pumped parts cleaner fluid. I've never had any 
trouble, but maybe that just means that I'm overdue.

The one case that I did get drilled and tapped was a fairly new case from a bus 
where something had come apart and disintegrated. I didn't want to take any 
chances on bits and pieces that might have been trapped in there, but those 
passages were pretty clean when I got the case back from RIMCO. They don't 
claim to do any internal cleaning, but they may have done some. I don't know.

Still, I suspect that this is what happened to you. Either that, or it was crud 
that came loose from inside the oil cooler.
