I think I can now answer my own question, after having messed with it some more. The procedure in the Haynes manual seems to be dead wrong. The right way to do it is to tighten the accelerator cable until it doesn't pull on the linkage, but no more than that. The result is that you really have to stamp on the pedal to get the kick-down to work. I am not sure whether this is right or not. I suppose it means that you really have to mean to do it; there's no chance of kicking down accidentally.

Does this sound right?
