The locking tab may be too worn.  Drill a small hole through the bolt head
and
thread a wire through it.  Wrap the wire around the torsion housing.  This
will
keep the bolt from loosening.  Look at the set screw the keeps the shift
coupler
connected to the tranny shift selector to see what I mean.
